Some Things You Should Know About Menopause & Hormone Therapy
What actually happens during menopause, why hormone therapy works, what the WHI study really showed, and what your treatment options look like.
A Brief History of Menopause Hormone Therapy
Estrogen Isolated
Scientists isolate and characterize estrogen. The first commercial estrogen product (Emmenin) becomes available, derived from pregnant women's urine.
Premarin Approved
FDA approves Premarin (conjugated equine estrogens), derived from pregnant mare urine. It becomes the dominant estrogen therapy for decades.
Estrogen Therapy Booms
Estrogen therapy becomes widely prescribed for menopausal symptoms. Robert Wilson's book 'Feminine Forever' (1966) promotes estrogen as an anti-aging treatment, driving massive adoption.
Endometrial Cancer Link
Studies reveal that unopposed estrogen increases endometrial cancer risk 4-8 fold. Estrogen prescriptions drop sharply. This leads to the addition of progestogens to protect the uterus.
PEPI Trial
The Postmenopausal Estrogen/Progestin Interventions trial shows that micronized progesterone preserves estrogen's HDL benefit while MPA does not, providing the first evidence that progestogen type matters.
WHI Bombshell
The Women's Health Initiative combined arm is stopped early. Headlines declare hormones cause cancer and heart attacks. MHT prescriptions drop 50-80% within months. An entire generation of women is denied effective therapy based on misinterpretation of data from women averaging age 63 on oral CEE + MPA.
Danish Study
The Danish Osteoporosis Prevention Study shows 52% reduction in cardiovascular disease with early-start MHT over 10 years. The timing hypothesis gains strong support.
ELITE Trial
Hodis et al. publish the ELITE trial in NEJM, the first RCT proving the timing hypothesis. Early-start estradiol slows atherosclerosis; late-start does not.
NAMS Updated Position
The North American Menopause Society updates its position statement: MHT is appropriate for symptomatic women under 60 or within 10 years of menopause. Individualized, formulation-specific approach endorsed.
FDA Removes Warnings
The FDA removes black box warnings for cardiovascular disease, breast cancer, and dementia from MHT products, acknowledging that the original warnings were based on misapplied WHI data. The endometrial cancer warning for unopposed estrogen remains.
What Your Hormones Actually Do
Menopause involves three hormones declining in different ways. Understanding each one is key to understanding your symptoms and your treatment options.
What Women Actually Experience
Menopause symptoms go far beyond hot flashes. Here's what research shows women actually experience, and how long it lasts.

Important: Vasomotor symptoms last a median of 7.4 years, not “a year or two” as commonly believed. The SWAN study found that 42% of women aged 60-65 still reported hot flashes. Genitourinary symptoms are progressive; they get worse over time without treatment and do not resolve on their own.
How I Like to Evaluate Menopause and Hormone Health
Menopause is often diagnosed clinically based on symptoms and age, but I believe in comprehensive lab work to get the full picture. Here's the panel I run.
My Lab Panel
FSH and LH
Follicle-stimulating hormone and luteinizing hormone. Elevated FSH (typically >30 mIU/mL) confirms ovarian failure. The FSH/LH ratio helps distinguish menopause from other causes of irregular periods.
Estradiol
The primary active estrogen. Levels below 20 pg/mL are consistent with menopause. During perimenopause, levels fluctuate wildly, so a single reading may not tell the full story.
Progesterone, Immunoassay
Confirms whether ovulation is occurring. Levels below 1 ng/mL indicate anovulation, which is one of the earliest signs of the menopausal transition.
Testosterone, Total, MS
Women produce testosterone too. Baseline levels help identify deficiency, especially relevant for sexual desire concerns. Also important before starting any testosterone therapy.
Sex Hormone Binding Globulin (SHBG)
The protein that binds sex hormones, controlling how much is “free” and active. Changes after menopause affect how much estrogen and testosterone your body can actually use.
DHEA Sulfate, Immunoassay
An adrenal androgen precursor. Helps assess adrenal function and provides context for overall androgen status. Declines with age independently of menopause.
TSH
Thyroid-stimulating hormone. Thyroid disease is common in midlife women and can mimic or worsen menopausal symptoms: fatigue, weight gain, mood changes, and hair loss all overlap.
T4 Free (FT4)
Free thyroxine. Combined with TSH, gives a complete picture of thyroid function. I check both because TSH alone can miss subclinical thyroid issues.
CBC (includes Differential and Platelets)
Complete blood count. Screens for anemia (a common cause of fatigue in midlife women), checks platelet count, and establishes baseline before starting therapy.
Comprehensive Metabolic Panel
Assesses kidney function, liver function, electrolytes, and blood glucose. Important baseline before starting any hormone therapy and screens for metabolic changes common after menopause.
Cortisol, Total
Screens for adrenal dysfunction. Chronic stress and cortisol dysregulation can amplify menopausal symptoms and affect hormone metabolism.
hs-CRP
High-sensitivity C-reactive protein. A marker of systemic inflammation. Elevated hs-CRP is associated with cardiovascular risk, which increases after menopause. Helps guide overall risk assessment.
Vitamin D, 25-Hydroxy, Total, Immunoassay
Essential for bone health, immune function, and mood. Deficiency is extremely common in midlife women and worsens the bone loss already accelerated by menopause.
The WHI Study: What Actually Happened
The Women's Health Initiative is the most consequential , and most misunderstood, study in menopause medicine. Understanding what it actually showed, and what it didn't, is essential.
Bottom line
In November 2025, the FDA removed black box warnings for cardiovascular disease, breast cancer, and dementia from MHT products, acknowledging that the original warnings were based on misapplied data. The science has corrected course. The fear hasn't fully caught up yet.
Treatment Options Compared
Hormone therapy is not one-size-fits-all. The formulation, the route, and the type of progestogen all change the risk profile. Here's what's available.
Transdermal Estradiol (Patches, Gels, Sprays)
Vivelle-Dot, Climara, Estrogel, Divigel, Evamist
Estradiol delivered through the skin, bypassing the liver entirely. This is the preferred route because it avoids the clotting, stroke, and metabolic risks associated with oral estrogen. Available as patches (changed once or twice weekly), gels (applied daily), or sprays.
Oral Estradiol
Estrace (micronized estradiol)
Oral bioidentical estradiol. While effective for symptoms, oral estrogen undergoes first-pass hepatic metabolism, which increases production of clotting factors, triglycerides, and inflammatory markers. This is why I prefer transdermal delivery for most women.
Oral Micronized Progesterone
Prometrium
The preferred progestogen for women with a uterus taking systemic estrogen. Unlike MPA (Provera), micronized progesterone has a more favorable safety profile and its metabolite allopregnanolone promotes sleep, a unique clinical benefit when taken at bedtime.
Vaginal Estrogen
Vagifem, Imvexxy, Estring, Estrace cream
Low-dose estrogen applied directly to vaginal tissue for genitourinary syndrome of menopause (GSM). Minimal systemic absorption means it’s safe for nearly all women, including many breast cancer survivors and women who cannot use systemic estrogen.
Vaginal DHEA (Prasterone)
Intrarosa
A vaginal insert containing DHEA (prasterone) that works through intracrinology, where the body’s own enzymes in vaginal tissue convert DHEA into both estrogens AND androgens locally. This dual-hormone approach addresses both the estrogen and androgen components of vaginal atrophy, without raising systemic hormone levels.
Non-Hormonal Options (for VMS)
Fezolinetant (Veozah), Paroxetine (Brisdelle), Venlafaxine, Gabapentin
For women who cannot or prefer not to use hormone therapy, several non-hormonal options exist. Fezolinetant (FDA-approved 2023) is a neurokinin 3 receptor antagonist that targets the mechanism of hot flashes directly. SSRIs, SNRIs, and gabapentin also provide meaningful VMS reduction.
Pellets (Hormone Implants)
Compounded pellets
Small pellets surgically implanted under the skin that slowly dissolve. While marketed as convenient, pellets consistently produce supraphysiologic hormone levels that cannot be adjusted or stopped once implanted. The 2019 Global Consensus on testosterone for women explicitly recommends against pellet therapy.
Important: Genitourinary syndrome of menopause
Genitourinary syndrome of menopause (vaginal dryness, painful intercourse, recurrent UTIs) is progressive and gets worse over time without treatment. Low-dose vaginal estrogen and vaginal DHEA (Intrarosa) are both safe, effective, and appropriate for almost all postmenopausal women, including many breast cancer survivors. Vaginal DHEA offers the additional benefit of local androgen production, which may improve sexual function beyond what estrogen alone provides. These are among the most underused effective therapies in women's health.
Why I Recommend Transdermal Estradiol + Micronized Progesterone
Not because it's the newest, but because the evidence shows this combination has the best balance of safety, efficacy, and quality of life.
Bottom line
For most symptomatic women within 10 years of menopause, transdermal estradiol combined with oral micronized progesterone at bedtime offers the best combination of safety (no VTE, no stroke risk, better breast profile), efficacy (equally effective as oral for symptoms, superior metabolic profile), and quality of life (sleep benefit, flexible dosing). This is the regimen supported by the strongest evidence.

Timeline
Most women notice improvement in hot flashes within 2-4 weeks, with full effect by 8-12 weeks. Sleep improvement with oral micronized progesterone is often noticed within days. Bone density effects take 1-2 years to measure. Vaginal estrogen may take 4-12 weeks for full benefit.
Safety, Side Effects, and Monitoring
Hormone therapy is generally safe when properly prescribed and monitored. The key is using the right formulation, the right route, and starting at the right time.
Who should be more cautious about systemic MHT?
- Women with a personal history of breast cancer (discuss with oncologist)
- Women with active cardiovascular disease
- Women with unexplained vaginal bleeding (evaluate first)
- Women with active liver disease (transdermal preferred if used)
- Women with a history of VTE (transdermal only, if at all)
- Women beyond 10 years past menopause (initiation generally not recommended)
Vaginal estrogen for GSM is appropriate for nearly all postmenopausal women, including many in the categories above.
The 2022 NAMS Position Statement, the most authoritative clinical guideline on menopause management, confirms that for symptomatic women under 60 or within 10 years of menopause, the benefits of MHT generally outweigh the risks. The 2025 FDA decision to remove black box warnings from MHT products reflects this evolving evidence.
